RISK ACKNOWLEDGEMENT
I understand that martial arts training (including Brazilian Jiu-Jitsu, Kickboxing, Kudo, Wrestling, sparring, grappling, striking drills, and conditioning) involves physical contact and risk of injury.

These risks include bruises, sprains, joint injuries, fractures, concussion, and in rare cases serious injury.

SPARRING AGREEMENT

I understand sparring and live training involve controlled physical contact. I accept that injuries may occur even with supervision and safety rules in place.

MEDICAL FITNESS CONFIRMATION *I confirm that I am physically fit to participate in martial arts training.

GYM RULES AGREEMENT *I agree to follow all gym rules and understand unsafe behaviour may result in removal from training.

GDPR DATA CONSENT *I consent to my personal data being stored for membership, safety, and communication purposes.

Privacy Policy Agreement *I have read and agree to the Privacy Policy.

MEDIA CONSENT *I give permission for photos and videos of me to be used for marketing, social media, website content, and promotional purposes without compensation.

FINAL WAIVER AGREEMENT *I confirm that I have read and understood this waiver and voluntarily accept all risks associated with martial arts training.
